FIRST-TIME PASSPORT APPLICANTS
First-time Applicants
If you are a United States citizen and do not yet possess a U.S. Passport book or card, you may apply for your passport through UGA Passport Services.
U.S. passport book – a globally accepted travel document regardless of mode of travel.
U.S. passport card – a wallet-size card only valid for land and sea travel between the United States, Mexico, Canada, Bermuda, and parts of the Caribbean
Under 16 years of age
Children under age 16 must apply for a passport in person with two parents or guardians. Passports for children under age 16 are valid for 5 years.
The following are required for the application process:
- Completed DS-11 form
- U.S. citizenship evidence for the child
- Evidence of parental relationship
- Identification for both parents
- 2” x 2” passport photo – photo services are available in UGA Passport Services
- Two separate forms of payment – (1) Personal check or money order made payable to the U.S. Department of State (2) Personal check, money order, or credit/debit card for fees payable to the University of Georgia

16 and 17 years of age
Applicants age 16 or 17 years of age must apply for a passport in person. Passports for applicants 16 and older are valid for 10 years.
