FIRST-TIME PASSPORT APPLICANTS​​

First-time Applicants

If you are a United States citizen and do not yet possess a U.S. Passport book or card, you may apply for your passport through UGA Passport Services. 

U.S. passport book – a globally accepted travel document regardless of mode of travel. 

U.S. passport card – a wallet-size card only valid for land and sea travel between the United States, Mexico, Canada, Bermuda, and parts of the Caribbean

Children under age 16 must apply for a passport in person with two parents or guardians.  Passports for children under age 16 are valid for 5 years. 

The following are required for the application process: 

  • Completed DS-11 form 
  • U.S. citizenship evidence for the child 
  • Evidence of parental relationship 
  • Identification for both parents 
  • 2” x 2” passport photo – photo services are available in UGA Passport Services 
  • Two separate forms of payment – (1) Personal check or money order made payable to the U.S. Department of State  (2) Personal check, money order, or credit/debit card for fees payable to the University of Georgia
